إجراء منظار علي القنوات المرارية في حالات ضيق حلمة الإثنا عشر ، مع بعض النصائح :
أثناء عمل منظار علي القنوات المرارية للحالات التي تعاني من صفراء إنسدادية مع ضيق حلمة الإثنا عشر نحتاج لعمل شق للعضلة العاصرة الصفراوية (عضلة صمام القناة المرارية) في حلمة الإثنا عشر لتصريف السائل المراري وقد نحتاج في بعض الحالات لعمل شق للعضلة العاصرة لقناة البنكرياس (عضلة صمام قناة البنكرياس) في الحلمة أيضا لتصريف عصارة البنكرياس حيث أن لكل قناة محور خاص بها، وقد نحتاج لأخذ عينة من الحلمة إذا كان هناك إحتمال وجود خلايا سرطانية في هذا المكان .

During ERCP for biliary drainage for cases of obstructive jaundice with papillary stenosis, endoscopic sphincterotomy of the choledochal sphincter is needed to facilitate free biliary drainage and sweeping of the CBD with balloon and to see the lumen of the most lower end of CBD. In some cases, papillotomy or sphincterotomy of the sphincter of the pancreatic duct may be needed for free drainage of pancreatic secretions . Sphincterotomy or Papillotomy facilitates endoscopic biopsy from the papilla if there is suspicion of dysplasia or malignancy .
