An 80 y old female patient presented with history of progressive jaundice for more than one month. Lab. investigations revealed markedly elevated serum bilirubin, 20 mg/dl mainly direct bilirubin. Abdominal CT scan was done as shown below and ERCP was done with insertion of a plastic stent for biliary drainage. The markedly dilated CBD could be appreciated from ERCP and pancreatic head mass is seen in CT images with dilated CBD and pancreatic duct. During ERCP an ampullary malignant looking mass was seen
