استكشاف القناة المرارية بالمنظار الجراحي لعلاج الصفراء الإنسدادية واستخراج حصوة كبيرة من القناة المرارية واستخراج دعامة وتنظيف القناة المرارية بإستخدام البالون .

Laparoscopic CBD exploration with Extraction of large CBD stone and a Plastic stent.This Patient underwent ERCP, one and half year ago, in another centre for treatment of calcular obstructive jaundice with plastic stenting of the CBD followed by Laparoscopic cholecystectomy in another Hospital. 6 months postoperatively, the Patient developed obstructive jaundice again. The Patient underwent ERCP again and they failed to extract the stone as it was large stone, and the obstructed stent was changed with a new stent, unfortunately the Patient developed recurrent symptoms after 6 months and developed obstructive jaundice again. MRCP revealed large CBD with a stent in the CBD.

The Patient was referred to us for definitive management.

Laparoscopic CBD exploration was successfully done with Extraction of a large CBD stone and the plastic stent with biliary clearance with a balloon catheter followed by primary closure of the CBD. The postoperative period was uneventful and the Patient was discharged on the first postoperative day.

The message is that, when there are large stones in the CBD , Laparoscopic CBD exploration during the same session of cholecystectomy is advisable, as ERCP may fail to extract large CBD stones and repeated stenting is a temporary solution of treatment .
