إستخدام المنظار الجراحي لإجراء عملية إستئصال المرارة ثم تفريغ وتنظيف كيس كبير صديدي بالبنكرياس ناتج عن مضاعفات إلتهاب حاد بالبنكرياس نتيجة مرور حصوات صغيرة بالقناة المرارية.

A 26 y old female patient presented with history of acute necrotizing Pancreatitis and calcular cholecystitis with multiple small stones in the gallbladder and stone passer with recurrent acute attacks of Pancreatitis. She was admitted to hospitals and treated conservatively for 2 months. She was still complaining of Abdominal pain and discomfort. A recent CT scan and MRI were done as shown revealing a large Pancreatic Cyst. Laparoscopic surgery was done for treatment of this large Pancreatic Cyst with Laparoscopic cholecystectomy in the same session. During Laparoscopy the retrogastric approach was chosen after lysis of adhesions and opening of the gastrocolic omentum. The sac was aspirated and part of the thin wall excised for histopathology, and the scope introduced inside the Cyst for aspiration and washing under direct vision. The wall at the floor was thin, infected , friable and vascular. Caution was taken to avoid bleeding. A drain was inserted. The postoperative period was uneventful and the patient discharged home on the 2 nd postoperative day for follow up.
