Laparoscopic treatment of infected immature large Pancreatic pseudocyst :

This short video demonstrates the value of Laparoscopy for treatment of infected immature large Pancreatic pseudocyst following gallstone severe acute pancreatitis . Limited dissection of the gastrocolic omentum was done to allow examination of the cyst wall through a retrogastric approach and aspiration of the cyst was done with a needle revealing infected contents .Then the cyst was opened and the scope introduced into the cyst cavity, the cyst was infected and the wall of the cyst was thin and friable necessitating external drainage. Complete Laparoscopic examination of the cyst cavity and its wall was done after complete aspiration of the infected contents as seen in this short unedited video, laparoscopy allows gentle debridement of the wall of this infected cyst. A biopsy was taken from the wall of the immature cyst for histopathology and external drainage tube was inserted into the cyst cavity.

Laparoscopic treatment of mature Pancreatic pseudocyst by cystogastostomy or cystojejunostomy is well established.

For infected immature large Pancreatic pseudocyst, as in our case, laparoscopy is a minimally invasive technique that does allow direct examination of the cyst wall and its cavity, to take a biopsy from the wall for histopathology and to ensure complete drainage with gentle debridement to achieve marked postoperative improvement in such critically ill patients.
